Sunshine Health has a special program for members with diabetes. Members in this program receive a Case Manager to help answer questions. They also work with you and your PCP. We want to help you gain better control of your diabetes.

The Case Manager can:

• Answer questions about diabetes
• Help you control it
• Help you get the supplies you may need
• Remind you about tests related to diabetes
• Work with your PCP to help meet your own needs

We will work with you and your PCP to help keep your diabetes from controlling your life.
