Medicaid Renewal

Don’t Forget!
You will receive your renewal information from the Department of Children and Family Services (DCF). You must complete the requested information. Go see your caseworker if needed. Tell your caseworker if you have had any major change in your life. This change could be:
- Address
- Telephone Number
- Family size
- Job
- Income
Your Medicaid eligibility must be renewed through DCF.
How do you complete the renewal process?
Sometimes, renewal can be done by mail. You need to follow the DCF process for doing so. You may be required to attend a face-to-face meeting. You must attend this meeting with your caseworker.
Please contact your county caseworker when it is time for your renewal.
What happens if you don’t renew your Medicaid
You can lose your Medicaid coverage. This will cause a break in your health coverage.
Verifying Citizenship
United States citizens and nationals must provide proof of U.S. citizenship. Every member of the family that is a U.S. citizen or national must provide proof. These items can prove citizen status:
- Birth Certificate
- Social Security Card
- Green Card
- Certificate of U.S. Naturalization
- School Records
- State Issued Picture ID
- Passport
- Confirmation of Birth
