Child Health Check-Up Program (CHCUP) is a preventive healthcare program for Florida Medicaid Members (Formerly known as EPSDT-Early and Periodic Screening, Diagnosis and Treatment). Sunshine Health offers this program to kids, teens and young adults under the age of 21.

Get a routine exam for your children. Well-visits cover complete health checkups. Well-visits are important for your children’s health. Your children may look and feel well but still have a health problem. Your doctor wants to see your children for regular checkups, not just when they are sick.

Infancy

• Birth
• 3 to 5 days
• By 1 month
• 2 months
• 4 months
• 6 months
• 9 months
• 12 months

Early Childhood

• 15 months
• 18 months
• 2 years
• 30 months
• 3 years
• 4 years

Middle Childhood

• 5 years
• 6 years
• 7 years
• 8 years
• 9 years
• 10 years

Adolescence

• 11 years and then every year until age 21
