Prior Authorization Requirements

Effective September 01, 2011, Sunshine State Health Plan® (Sunshine Health) has revised the prior authorization requirements for Medicaid Members.  For more information click here.

Specifically the following changes will occur:

Removed
• Nuclear Cardiology

Added
• Hospital Outpatient Surgery (Must be requested by ordering physician)
