Dear Sunshine State Health Plan Provider:

Sunshine State Health Plan is making some important changes to our Preferred Drug List, and we are writing to communicate the changes to you. The changes will be effective September 24, 2010, a summary of these changes can be found on our website www.sunshinestatehealth.com.

If you, as a physician, feel that a patient will require medication therapy outside of the described guidelines, you may submit a prior authorization request to US Script, the pharmacy benefit manager for Sunshine State Health Plan. The Medication Prior Authorization Request Form is also available on the Sunshine State website, www.sunshinestatehealth.com, under the Provider Forms section. This form may be faxed to US Script at 1-866-399-0929.
