To sign up to be part of the Sunshine State Health Plan (Sunshine Health) network, you will need to complete an Application Packet, which includes a Provider Agreement and Credentialing Application.
For your convenience we have also included a checklist to guide you through the process of completing the Credentialing Application. If you are registered with CAQH please download and complete only the CAQH Provider Data Form. Once you have completed the appropriate material, please fax it to Sunshine State Health Plan at (866) 796-0540 or mail to:
